This referral form is strictly only to be completed by a general dental practitioner who has already carried out a pre-assessment on the patient and can provide the required information.

Please be aware of the below points before completing an online referral. Unfortunately, if the referral does not provide the required information, it will be rejected:

• The patient is too young for an orthodontic assessment and there is no clear clinical justification for an early referral.
• Important clinical information is missing.
• There is no clear indication or reason for the orthodontic assessment.
• The information provided is insufficient to determine whether an assessment is appropriate.
